Please read through the waiver below carefully and check the boxes below with your child if you both agree.

Parental Consent/Release: I give permission for the above named child to participate in Harvest Bible Church's Youth Ministry Programs during the 2019-2020 ministry year (June 2019-August 2020). I release Harvest Bible Church Windsor, its agents, staff and volunteers from any liability.

Student Consent/Release: I, the above named child, am registering for participation in Harvest Bible Church Windsor's Youth Ministry Programs during the 2019-2020 ministry year (June 2019-August 2020). I release Harvest Bible Church Windsor, its agents, staff and volunteers from any liability

Photo Release & Consent: I hereby grant permission to Harvest Bible Church Windsor to use images of the above named child on its web site or other church social media platforms or publications without further consideration. I understand that no names will be used online or in publications unless specific written permission is given to an appropriate staff member. I understand that once any image is posted to Harvest’s web site the image can be downloaded by any computer user around the world. I hereby attest that I am the legal parent or guardian of the child listed above. This consent is effective until such time as I revoke it in writing.

All parts of section 2 needs to be filled out once per household (not for each child unless the information is different).
